Accessing Health Records
Patients are entitled to be given access to all personal information held by us. Copies of health records can be released under the Health Records and Information Privacy Act 2002 and Privacy and Personal Information Protection Act 1998 or the Freedom of Information Act 1989. Information in accessing your personal health records is provided below. See the Freedom of Information page for further information on this process.
Application for your Health Records
You are entitled to request access to all personal information, including your Patient Health Care Record, held by us. If you are not the patient then you will need to include written consent from the patient clearly indicating that we can disclose the record to you. Normally you will be asked to apply for access in writing and provide identification. You may be charged a fee if you request copies of your personal information or Patient Health Care Record.
Access to your personal information may be declined in special circumstances, such as where giving access would put you or another person at risk of mental or physical harm.
If you believe the information we hold about you is incorrect or an error has been made, please let us know and we will correct the information or add a notation to your record.

The Health Care Complaints Commission is an independent body set up to look at people’s complaints about health services. A complaint may be investigated by the Commission, referred to another body or person for investigation, referred for conciliation with the complainant’s permission or referred to the Director-General of Health.
